For Insurance Claims Copy of Medical Letters, Discharge Summary Sheet, Billing & Invoices are Issued Once Treatment Is Completed & Full Payments Are Made.
Miswak Dental Hospital is NOT responsible for any failure of an insurance claim
MDH will NOT Issue any Letters if the Treatment is Incomplete / Payment Not Paid / False Personal Information / Treatment Plans & Payment Amount.
